TO WHOM IT MAY CONCERN:
Mr. John Doe has been under my care for two years
for the treatment of Vitiligo. He’s had Vitiligo for many years
and the only modality to which he has responded consistently has been PUVA
treatment. This treatment consists of internal medicines called psoralens
followed by Ultra Violet A treatment (UVA). The internal medicines given as part
of the PUVA treatment can have internal side effects. In addition, they can
cause significant nausea, which the patient has been experiencing.
Narrow-band Ultra Violet B treatment can produce similar
benefits to PUVA without the internal side effects.
Further, it can be provided at home, significantly increasing the likelihood of
compliance on the patient’s part. This is likely to cause a quicker resolution
of the problem, which will be beneficial not only to the patient, but to his
insurance company. This treatment will probably need to be undertaken for
several months at a time throughout his life to maintain a state of remission. A
home unit is medically necessary since treatment in a physician’s office would
require a two-hour round trip three times a week.
Keeping these factors in mind, I’m prescribing the National
Biological Corporation Panosol II UVB Narrow Band for him. This unit is FDA compliant, and has the same healing
potential as the ultraviolet units used for treatment in phototherapy centers,
but was designed specifically for home use. It is easy and safe for the patient
to operate after proper instruction and continued monitoring by me.
